PAPERWORK DUE BY NOVEMBER 30TH, 2004 Items "For Your Information" - to keep* / not to be turned in* Important Dates / Mandatory Events * Information Sheet (Facts, Deadlines, Requirements, Rehearsals, Pageant Info) * Pageant Informational Flyer * Platform Information Sheet [How to fill out the Platform Questions form]* Production Number Information * Rehearsal Location * Swimsuit Guidelines * Contact Names - Board Items to be filled out and returned to Pageant by Deadline_____ Pageant Application _____ Fact Sheet for Judges' Interview (return 1 original, 10 copies)_____ Platform Questions Form (return 1 original, 10 copies)_____ Advertising/Sponsorship Request Form (make copies to give out!)_____ Non-Profit Letter from NFSO (make copies to give out to potential sponsors/advertisers)_____ Talent Request/Info Form _____ Sample Index Cards: (1) Swimsuit card, (2) Talent card, (3) Evening Wear card _____ Confirmation of Knowledge Regarding Rehearsals, Tickets & Ads, Sponsorships Other Items/Requirements to Be Turned in By November 30th, 2004_____ Photography to be turned in (no crown) - can be color or black/white (color suggested for the judges' interview books), Interview suit suggested. Two 5 x 7 headshot photos (good quality photo)_____ Birth Certificate to be turned in (ASAP/NOW)_____ (2) Proof of Residence to be turned in _____ Two Cassette Tapes or CDs for Talent competition (no longer than 2 minutes)_____ Any sponsorships or ad sales TICKETS DUE BY JANUARY 2ND, 2005 Tickets will be made available to contestants approximately 2 months prior to the pageant to sell to friends and family at a discounted rate ($12 in advance through 1/2/05, $15 at the door). All ticket moneys or unsold tickets MUST be returned to the pageant by 1/2/05 at the last rehearsal - NO EXCEPTIONS. Just selling 10 tickets is a $30.00 savings this way! Check out tickets from the directors as early as late October or early November 2004.
